· Schedule: Monday and Wednesday (Operating Room)7a-4p, Tuesday and Wednesday (In clinic) 7:30a-4p and Friday 7:30a-4p (Friday is scheduled based on patient volume)

· Practice Setting: Outpatient

· Type of cases: Surgery and Clinic Appointments

· Credentialing timeframe: 90 days. Temporary privileges are available.

· Electronic Medical Record: MediTech

· Certifications required: Board Certified, Basic Life Support (BLS), Advanced Cardiovascular Life Support (ACLS) and Drug Enforcement Administration (DEA)
